To obtain one of our Communicator speech devices through your Insurance company, please complete the following steps...

  1. 1.Complete the “Client Information” form (click link below)

               View/Print “Client Information” form

  2. 2.Obtain a report/recommendation for a SGD (eg. Communicator U1) from 
    a Speech Language Pathologist (SLP)

               Download Blank SLP Report (*.doc format)

  3. 3.Obtain a prescription for the SGD from the treating physician.

               View/Print Physician Prescription form

  4. 4.Photocopy both sides of your insurance card.

  5. 5.Fax all of all the above documents to 360-715-9633
